Tube failure of the heat exchanger E-20-128

    Abstract

    Based on inspection and measurements of tube samples, the root causes of tube failures in the heat exchanger E-20-128 have been assessed. The perforating damage on the external surface of the failed tube is a round 2mm hole that originated from an internal corrosion pit. The tube material shows otherwise no obvious deficiencies, but the inner surface is covered by relatively thick blistered oxide. The thick internal oxide that was formed at relatively high temperature appears to point to fabrication-related deviation from the intended tube condition.
